Plan for the Future

The transition from youth to adulthood is an important time—especially for young people with disabilities or long-term care needs. The ADRC is here to help families plan for the future by providing guidance on education, employment, independent living, and eligibility for adult long-term care programs. Our goal is to ensure a smooth and supported path into adulthood. Each step of the Roadmap to Adulthood explains what steps ensure a smooth transition.

Youth Turns 17

Around the youth’s 17 birthday is a great time to start planning for the transition to adulthood. The transition will begin with a referral to an ADRC Specialist who will be your guide throughout the process. Contact the ADRC to request this referral!

Meet and Greet

An ADRC Specialist will meet with the youth and legal guardian. This meeting is to get to know the youth, as well as to provide information about the process over the next year.

Functional Screen Visit

An ADRC Specialist will meet with the youth at the location of their choice. During this visit the ADRC Specialist will complete an assessment to find functional eligibility for an assistance program.

Referral to Disability Benefit Specialist

As needed the ADRC Specialist can make a referral to a disability benefit specialist to aide in, if needed: Learning public benefits, applying for benefits, appealing denied benefits, and working with partner agencies. They can also assist with obtaining financial eligibility for adult Long Term Care Medicaid.

Referral to Financial Eligibility

The ADRC specialist can make a referral to Waupaca County Economic Support if needed. They can assist in obtaining financial eligibility for adult Long Term Care if the youth is not already enrolled to a full benefit Medicaid Program.

Enrollment counseling

ADRC specialist will meet with the youth and legal guardian once found functionally and financially eligible. This meeting will be to make a decision on which Long Term Care program to enroll with IRIS, Family Care or Partnership.
